(ii) county* of residence in Ireland / or select 'international' if outside the Republic of Ireland, (iii) your e-mail address*,

Click on the LOGIN icon, using your registered email address and chosen password.

Click on the MANAGE CHILDREN icon to add your child to your profile.

Click on the PAY NOW button and you'll be taken to a secure on-line PayPal payment page. The cost of using the screening tool is €9.99 and access to the full questionnaire will be made available for 90 days.

Click on the START HERE icon, select your child's name and start completing the questionnaires. It is recommended that you start with the questionnaires that match your child's developmental concerns.
After each questionnaire select the FINISH button and you will receive a copy of your child's results in print-ready format and if selected at the time of registration, a copy will be e-mailed to you and your GP (also optional).
*Why county of residence?
The aim of the software is to provide parents with a list of local professionals (closest HSE health centre, Assessment of Need Officer, or private therapists/medics) if a need for an onward referral has been identified.
*Why your e-mail address?
Once you have completed the questionnaires, a copy of the assessment results will be e-mailed to the e-mail address you provided. If you do not wish for a copy to be e-mailed to you, select the PRINT function and print a PDF copy of your child's results.
*Why your child's GP's e-mail address?
Best practice suggest involving your child's GP in all aspect of health and development. GPs are uniquely qualified to perform medical evaluations and explore the reason for the delay. GPs also have all the important information, such as whether there is already a referral in the works, or if the child additional health information (such as a child born prematurely, or requiring extended hospitalisations) is not in fact delayed when development is corrected for prematurity or time when he/she was unwell. The GPs are also the gatekeepers of the primary care system and in the best position to make onward referrals to the relevant professionals for further diagnostic testing.
*What do you mean by electronic communication consent?
